Request an AppointmentFiguring Out Which Appointment You Need
If you’re having trouble determining what type of appointment to schedule, the information below can help, or feel free to call our office at (904) 259-5007 with any questions.

If you’re not yet an established patient and haven’t had routine appointments with our office, we can still see you for an emergency visit. You’ll be scheduled for a limited examination and x-ray to address a single tooth issue. This appointment type does not establish you as a patient at our office, so you’d be charged for any further toothache appointments needed in the future. If you have multiple teeth issues, it’s more cost-effective to become an established patient so we can address all of your concerns at once, which you can start by exploring our full range of dental services.
Insurance
Our office is an out-of-network provider with every insurance company. This means that for us to bill your insurance, your plan must be a PPO plan with out-of-network coverage. We do not accept Medicaid or Medicare plans. Please call our office with your insurance information, and one of our front desk specialists will be happy to look up your benefits and answer any questions you may have.
Billing and Financial Options
We understand that paying out of pocket for dental treatment, whether it’s routine general dentistry or more involved restorative dentistry, can be a concern. That’s why we offer financial options that allow our patients to pay over time for the treatment they need now. Please call our office for more information, or to get pre-qualified today for your upcoming treatment.
Online Scheduling and Patient Forms

Electronic Forms
At Baker Family Dental, we offer patients the ability to fill out their forms online rather than in the office on the day of their appointment. This lets you complete them from the convenience of your own home or office and reduces your waiting time once you arrive.
